United Healthcare Insurance Accepted

Phoenix Spine and Joint can help determine if your plan will cover your procedure and estimate your out-of-pocket cost. We are an In-Network provider with United Healthcare.

UnitedHealth Group Incorporated is an American multinational managed healthcare and insurance company based in Minnetonka, Minnesota. It offers health care products and insurance services.

In the United States, UnitedHealthcare offers health benefit programs for all ages and lifestyles. This includes individuals, employers, and Medicare and Medicaid beneficiaries.

In most cases with UnitedHealth Insurance, when you go to a network provider like Phoenix Spine & Joint, you will not need to submit a claim for your care. However, there are a few occasions where submitting a claim on your own may be required, in order for you or the provider to receive reimbursement.

After your claim is processed, you’ll receive an EOB (Explanation of Benefits). This explains the charges applied to your deductible (the amount you pay for covered services before your plan begins to pay), plus any charges you may owe the provider.

How much will the surgery center cost under my plan?

Your out-of-pocket cost for the Surgery Center depends on your plan and how much you’ve already spent this year. For most United Health Care policies your responsibility is the sum of your remaining deductible and your co-insurance up to your out-of-pocket maximum. To estimate your Surgery Center related out-of-pocket cost (expense) for a procedure under your United Health Care insurance please complete the following:

Policy Deductible ($) i the amount you must pay out of your pocket each year before your insurance benefits kick in. Until you reach your deductible your insurance pays zero. Deductibles can be individual or by family.
Deductible already met ($) i the amount you have paid this year toward your deductible.
Out of pocket (OOP) maximum ($) i the most you can be expected to pay each year. After you reach your out-of-pocket maximum your insurance picks up the remainder of the tab.
OOP already met ($) i the amount you have paid toward your out-of-pocket maximum. This includes co-insurance over your deductible.
Co-insurance (%) i the percentage of charges you are responsible for above your deductible but below your out-of-pocket maximum.
I have no idea, just use some typical numbers to give me a rough idea

Your out-of-pocket cost: $0

Insurance is complicated, we can help.

*If your policy has no out of pocket maximum, or your deductible is per service, then this calculator won’t work for you. Remember also that there are separate charges for the surgeon as well as the anesthesiologist which are not included in this calculator.
